i dont have anything decent to add (like i ever do after you have posted your excellent and thorough ideas), but i will toss out a couple of things (which you likely thought of and omitted for a very good reason). I have not had many idle issues, so I dont have a lot of experience with 'fixes' other than helping friends out.
the idle bypass plates - they work for some, don't for many. might be another option for folks.
a note about some aftermarket IAC's installing upside down might be good - some folks dont read the little piece of paper that comes with them, which note this.
i dont have nearly the resources that you do in terms of known cool internet links, but here is one article which is fairly decent (i dont agree wtih a couple things), but it shows the general idea and has pics (Pics are good. LOL).
