I got the handle and o/d button from MGW in the link trublustallion posted. The hardest part about installing it was removing the stock handle. The directions said to twist and pull up gently on the stock shift knob until it comes loose. Well mine never did really break loose so I gave it a pretty good twist and pull and it popped right off tearing the wires off with it. Luckily for me there was enough slack left in the wires that it turned out to not be a problem. Other than that installation was pretty straight foward.
