I have the following parts to setup a hydraulic clutch. I used the same kit for a T5 conversion on my '66 and have loved it. I have a second set of these parts I had intended to use but no longer have that project.
CNC 711-3/4 master cylinder without resivour - $85 new
CNC 305-B push slave cylinder - $65 new
CNC 1312 adjustable rod - $18 new
Longacre 28050 Hydraulic Clutch Line 36" - $32 new (missing short hard line)
Asking $100 for all.
I used a T5 bracket I got from DazeCars which worked out great.

He also has a pretty cool looking push-rod for the pedal to master cylinder. I made my own.
I used a T5 bell housing and a Centerforce dual-friction clutch and it works smooth with a medium pedal.
