Worn clutch assembly. At 90k miles on the original clutch it's probably near the end of its life.
As it's a catted H-pipe I don't think you'll have any problem with emissions. The BBK TL shorty headers produce a 5-7rwhp gain.

Try scanning the ECU for codes and see what you come up with. It'll help in troubleshooting the problem.
There's indeed a chance you might have a "no tune required" CAI, in which case your ECU may still have the stock tune anyway.

Sounds like a problem with the tune especially for cold start, open loop operation.
Nope, but others seem to swear by it. Admittedly it doesn't come cheap though.
Your best option would be to have the headers ceramic-coated.
