01 GT Hesitation?

I am going to try and explain this as best I can. 2001 GT, 100,000 miles. At low RPM's it has very bad hesitation, kind of like it is starving for fuel. It doesn't do this as bad if you barely press the gas. It is also much worse at lower RPM's and gets better the higher in the RPM range you get. I have 2 dummy lights on, Service Engine Soon and Check Fuel Cap. I had the code read at AutoZone and it says that fuel is evaporating out of the gas tank. I have replaced fuel injectors and that did not fix the problem.

Any ideas?

Do a plug and fuel filter change if they haven't been done in any recent time and clean the MAF sensor. Good PM even if it doesnt fix the problem.
May need a new gas cap too if the one you have is venting to atmosphere.
 
if it feels like it is running lean off-idle, first step is to check your fuel pressure. I've seen plenty of pumps start to fail by first simply having insufficient pressure, and later taking a complete dump and failing.

if pressure looks good, then I'd look for problems in the intake, from the MAF on...
