Just get 3/8" and 5/16" plugs from any fastener supplier, Lowe's, HD, local bolt & screw supply and press them in. You can even find chrome ones if you look around... I had mine filled and painted along with the holes behind the blue oval... and love the look...

I've been having this problem lately... I had it once before, but it quit. The pedal will just get real sloppy for no apparent reason, but after pushing the pedal to the floor several times it goes back to normal. I can only assume that whatever mechanism they use to take up slack in the cable...
