2011 Gt Fouling O2 Sensors

I about 6 months ago I purchased a 2011 GT with headers, air intake, among a few other things. I had to replace the oxygen sensor after purchase bc it was making the check engine light come on. Replaced it no problem, and not a few months later that same thing is happened again. Sensor appears to be covered in carbon. Do you think the headers and air intake are causing the car to run rich and fouling the sensor? Seems odd to me that it would fail twice so close together.

Was the car tuned for the headers and intake? If not then you need a tune.

as far as I know it was not, Ill look into getting one.
