Major Clutch problems GT

I am just curious if anyone else has had this problem with the GT clutch. I was driving my car when I noticed a metal lathe sound coming from the bellhousing whenever I put the car in gear and had the clutch down. I tore into it and found out the clip that holds the fork clutch to the pivot ball in the tranny had broken off and the clutch fork had fallen and tore the throw our bearing out and somehow caused the clutch disc to bust and one of the springs to pop out and dig into the pressure plate and was not allowing the clutch to disengage. I was not even able to get it put in gear without grinding. I was just wondering if anyone else has had this problem. BY the way my car is a 2000 mustang GT 5 speed with the T-45 tremec.
