NEED HELP!!!antone had this problem?

I have a 1992 mustangand when I shift gears the engine reves up like I have my foor on the gas, the car goes into gear when I shift but its not a smooth shift any ideas on whats wrong could I need to add transmission fluid or have it flushed and put new in or something like that

slipping clutches, check the fluid for a burned smell and a brown color. rebuild time.
