What is needed for a rear disc conversion? Has anyone did it successfully?

Hey guys...I did a search and came out empty handed as usual. Can somebody tell me what is need for a rear disc conversion and out of what car(s) can I get it...I'm looking for a junk yard setup. This is what I think: please add or correct me on things listed below:

axles, calipers, caliper mounting brackets, pads, brake lines, flat mounting brackets, anti-moan brackets, and mounting bolts. Did I miss anything? Do I really have to change the axels? What esle would I need? How hard of a job is it and has anyone done it before? Is there a rather cheap "kit" out there besides the junk yard to do the job?

i had a friend who got a complete rear end out of a thunderbird, already had 355's and rear disks, and he went from there, this was back in the day when all these newfingled thingmabobs weren't available yet. But it is possible and pretty cheap out of a junk yard.
