    I may not fully understand the question but if there's an extra hole in the deck without a matching channel in the head, and the head gasket covers up that extra hole, I don't think the coolant has anywhere to go so you wouldn't really need to do anything about it.

    Mine did that some years ago, it was a grinding / whining. Turned out that ATF in a manual transmission actually needs to be changed every 20 years or so. I also had a u-joint go dry and seize up pretty good, that gave me some shudder and some noise too.
