Rather than a cam, the rough idle may just be the timing off, that's what was up with my 5.0 when I bought it. I set it back to 10-12 and it smoothed out and idled much better.

just make sure the hi output alternator makes adequate amperage at idle rpm. I bought a 180 amp alt for my s-10 that performed worse at low rpm than my stock 100 amp one.
