Gas gauge doesn't work.

My gas gauge has been acting up for a while now, it will read accurately when i first start the car up, but when i start driving it gradually drops down to empty and will stay there. Sometimes when I get on it hard the needle will move back up to where it should be but that doesn't last long. Anyone else experience this issue?
