Flex-A-Lite black majic wiring question

well i bought the car with the fan previously installed and my friend unplugged the wires going to it without looking to see what goes where. so i was wondering if anyone could tell me what the outouts on the fan are to, i believe there are 5 out puts 4 on top one on the bottom... also i have 3 wires going to it if that helps at all and the temperature wire.
